GPs on Curzon promote Care plans to eligible patients. We have a wonderful Chronic Disease Nurse who sees patients as part of their chronic disease care plan and can assist with education and organisation of many services. We provide a folder for all patients to help organise their results, testing and specialist information. The patient also gets to have a review with their usual GP.
